We are a team of Physicians and Nurse Practitioners with the focus on caring for patients in the transition home after surgery. Dr. Gaylor and Dr. Stewart have worked in acute care for many years with a focus on post operative orthopedic care. Both were deeply involved with not only trying to improve the care provided in the hospital, but also looking at the issues that occur once people are discharged to home after surgery.
With this insight they felt better patient care could be provided by also having close contact with patients once they returned home. This became what we refer to as Transitional Care medicine. Our goal is to help patients transition from the hospital to home in the most elegant way possible. While we hope to minimize complications, we also want to improve the patient experience and outcomes.
Our team follows patients once discharged to home and is available at all times, including weekends. If any issues arise we want to be informed. We will continue to co-manage all issues with the patient’s individual surgeons. As patients progress towards a full recovery our goal is to help transition patients back to their primary physicians as the acute surgical conditions resolve.
