Dr. Gaylor and Dr. Stewart first started the Transitional Care Medicine program in October of 2020. Our goal is to help transition patients home after surgery and improve the patient experience. When any concerns arise we want to be contact to help manage your care.

We understand that this is a major event in your life.  A big decision has been made when you decided to have surgery.  We recognize that there are a lot of unknowns for you post-operatively.  We are ready to help guide you through the transitions.  As part of your team,  we have cared for thousands of Orthopedic patients and are here to help you navigate through this new and exciting part of your life.
